ASSIGNMENT #1 - HEALTH AND SAFETY CHECKLIST
Health and Safety on the job is of the utmost importance. Therefore, before commencing work, you must review the health and safety regulations of the workplace with your supervisor.



ASSIGNMENT # 2 - "THE FIRST DAY"

The first day on the job is filled with excitement and apprehension. It is very important that you make a good impression. You want to stand out from the crowd.

ASSIGNMENT #3 - SAFETY ASSIGNMENT
Safety is a big concern in many different types of jobs. Acquaint yourself with the safety procedures and concerns of your placement.

ASSIGNMENT #4 - SPOTLIGHT ON MY CO-OP
This assignment highlights your achievements at your co-op placement. It will be featured in front of the co-op room to promote and educate students and staff about the Cooperative Education program here at the Youville Centre.

ASSIGNMENT # 5 - PERSONALIZED PLACEMENT LEARNING PLAN WORKSHEET

FITTING IN CHECKLIST
The purpose of this checklist is to give initial feedback to students after they have worked at their co-op placements for three weeks. This evaluation is to be completed and signed by both the employer and the student.

ASSIGNMENT #6 - ANIMOTO VIDEO ASSIGNMENT
Your objective for this assignment is to make a 30 second video about your co-op placement that you can show to the class.

ASSIGNMENT #7 - EMPLOYABILITY SKILLS PORTFOLIO
An Employability Skills Portfolio is a personal record of accomplishments. It is your life and learning. The documents and articles are your personal choice, but should reflect your true abilities, skills and accomplishments. It is why you should be hired for the job you want or accepted at the college or university you want to attend.


ASSIGNMENT # 8 - CO-OP PLACEMENT POWERPOINT

As a Cooperative Education student, you are responsible for a PowerPoint presentation to showcase your cooperative education work site. Your completed project must contain a minimum of 10 slides and 4 photographs.

ASSIGNMENT # 9 - EMPLOYER THANK YOU LETTER
You will write a thank you letter to your co-op supervisor and to deliver it to him/her in person on the last day of your co-op placement. Please follow the proper letter format as outlined in the assignment.

ASSIGNMENT #10 - EXIT RESUME
Prepare an updated resume which includes your co-op placement.

EMPLOYER EVALUATION - (MID-TERM AND FINAL)
Your employer will evaluate your work habits, attitude, initiative and otheressential employability skills. Your evaluation sheet must be handed in midway through your co-op placement and then again at the end of your placement.

SELF REFLECTION AND EVALUATION
The purpose of this evaluation is to allow you, the student, to evaluate yourself in the aspects of the job that you are being successful at; as well as those which require improvement. Be honest. This evaluation is between you and your teacher monitor.

WEEKLY LOG SHEET
Your log sheets must be handed in every Wednesday during our mandatory Wednesday meetings. Make sure both you and your employer have signed it.

LOG TOPIC SHEET
On the reverse side of your log topic sheet there is a reflection section. For your first log, you will need to answer question #1. For log #2, answer question #2 and so on.
